Important Information on How to Submit
NPI Numbers to HCAS Health Plans
The National Provider Identifier (NPI) is a unique, government issued
standard identifier mandated by the Health Insurance Portability
and Accountability Act of 1996 (HIPAA). The purpose of the NPI is
to improve the efficiency and effectiveness of the electronic transmission
of health information.
All providers must submit their NPI numbers to each health plan
in which they have a contract.
Listed below are the website links (where available) and general
instructions on how to submit NPI numbers to each of the HCAS participating
health plans:
Blue Cross Blue Shield of Massachusetts:

Fallon Community Health Plan:
1. Online submission: https://www.fchp.org/Extranet/Providers/Secure/submitNPI.aspx
2. Send to your Provider Relations Representative and include name,
address, FCHP legacy number and NPI number
3. Fax to: 508-368-9902. Include name, address, FCHP legacy number
and NPI number
4. Include NPI number on your credentialing application submitted
to CAQH
